Flu Shots
The UT Student Health Center Pharmacy will be offering flu shots on-site.
Shots will be free to employees enrolled in the state’s health insurance plans and who bring a copy of their CVS/Caremark prescription card.
Please complete the shot waiver form and informed consent form below and bring both to the benefits fair.
Cash, check or card will be accepted from employees not enrolled in the state’s health insurance.
